Paperwork

  • Please COMPLETE AND PRINT OFF OFA forms to bring to appointment. Forms can be accessed at this link: https://ofa.org/applications/
  • There will also be forms available at the Health Clinic.
  • Submitting test results to OFA is client’s choice and at their expense.  See link to OFA fees below.

 

Resources
  • Both veterinarians provide the paperwork for the services which clients will receive at check-in. Whomever brings the dog(s) to the Health Clinics will receive a copy of the results.

HEALTH CLINIC SERVICES, LOCATION & TIMES
 
Thank you for testing and contributing to the health and well-being of your breed!  The 2024 Winter Cluster is offering the following Health Clinic Services provided by the following veterinarians.
 
Eye Clinic - Dr. Paul Scherlie, DVM, DACVO, NWVS Ophthalmology
 
Heart Clinic - Dr. J. A. Woodfield Diplomate, ACVIM, Northwest Cardiology Consultants
 
Thank you for visiting the Winter Cluster Health Services webpage.  Feel free to bookmark and/or share these links with owners and families coming to the Health Clinic.
 
  •  Health Clinic Reservations MUST BE SUBMITTED AND PRE-PAID via the ONLINE FORM.
  •  Select the services by 1) number of dogs; 2) breed, and 3) preferred day for service.  If choosing more than one service, we will attempt to provide services on the same day, and as close in the schedule, if possible.  Complete ONE FORM for each breed.
  • Note:  No actual appointment time(s) will be set now. We will email all paid clients a scheduling calendar AFTER the Judging Schedule is published by BaRay.  This usually occurs about one week prior to the Show (approximately January 5, 2024).  This allows clients an opportunity to review their breed ring times prior to scheduling their appointments.
  •  Once the Reservation Form is complete and submitted, use the payment link that pops up to pay for services with a debit or credit card.  Note: only PRE-PAID reservations will be considered for scheduling.  If you need assistant with you online payment, please email the Cluster Treasurer (pvdftreasurer@gmail.com)
  • After payment, an auto-generated confirmation will be sent by email.
  •  Note:  Pre-payment DOES NOT guarantee services.  The attending veterinarians and volunteer staff will do their best to accommodate all client’s schedules, however appointments will be limited by Clinic hours and available appointment slots.  All services not provided will be issued a refund LESS any merchant fees.    
  • Health Clinic Services are offered as a courtesy to breeders, owners, and exhibitors of the Western Washington Winter Cluster Dog Show.  The scheduling and appointment format of this Clinic may not meet the needs of all clients.
  •  Daily schedules are subject to services taking longer than expected.  Please plan to be at the Health Clinic at least 45 minutes and up to 2+ hours.
  • Health Clinic Reservations close Wednesday, December 27, 2023, or earlier, if sold out.  If any appointments remain after the closing date, they can be purchased as a “walk-in” client at the Health Clinic in the ShowPlex on Saturday or Sunday (January 13 and 14, 2023).  A waitlist MAY also be created.
  • Please arrive at least 20 minutes before your scheduled appointment.  Enter through the Orange Gate and park in designated parking spaces on the northwest side of the Showplex.  Access link to Fairgrounds map here:  https://pvdf.org/maps.htm
  • By submitting this reservation request, users agree to all show rules and requirements as stated on this reservation and in the Premium (whether they’ve read them or not).
Reservations for Health Clinic (Heart and Eye services) at the Western Washington Winter Cluster Dog Show are now closed.  If any available appointments remain, they can be purchased at the Clinic at the Show, starting Saturday morning (Jan 13, 2024) at 9:00AM.

HEALTH CLINIC APPOINTMENT-SCHEDULING UPDATE   9:00 PM, Sunday, January 7, 2024
On January 6 & 7, 2024, emails were sent out for people to schedule their Saturday and Sunday Health Clinic appointments.  The email was sent to those people who have already MADE and PAID for Health Clinic Services.  If you are one of those people, and have not seen your email yet, please check your INBOX, JUNK and/or SPAM folders.  If you still don’t see it there, please email:  pvdfhealthcheck@gmail.com or pvdf.healthclinic@gmail.com.
