CARDIAC: Dr. J. A. Woodfield Diplomate, ACVIM (Cardiology) NORTHWEST CARDIOLOGY CONSULTANTS Genetic Heart Screening – $60.00 per dog • Echocardiograms – $250.00 per dog
EYE: Dr. Paul Scherlie, DVM, DACVO – NWVS Ophthalmology Genetic Eye Screen/CERF exams – $45.00 per dog

The health clinic schedule is full! All who have paid are scheduled and your time(s) have been emailed.

- Please arrive at least 20 minutes before your scheduled appointment.
- If you have a conflict with your health services and any ring times, please come to the health clinics to be worked back into the schedule. Please bring your patience and time.
- Please Park in designated parking spaces inside the gates across the roadway from the Health Clinic in the stadium parking area.

- Both vets will be providing the OFA forms. You are welcome to come by earlier to pick up the forms or when you arrive at least 20 minutes you can fill them out at that time. We apologize for the posting re: downloading forms from the OFA.
- You will need to provide: the dog's registration number, owner's name and contact information, dog's registered name, date of birth, sex, breed, and permanent identification.
- If you are supporting pet owners, please share all of this information with them.
