Eye and Heart services are being offered!

  • Sunday, January 15th from 9 to 3 pm

Process for obtaining either or both services for one/multiple dogs:

Complete and submit the Health Clinic Information Form  - 

 

 

 

You will be asked to enter in the name and cell phone numbers of who is paying and who is bringing the dog to the health clinics.

  • You will be able to indicate your preferences by date and window of time for each type of service.
  • You will be able to pay by credit card at the end of selecting your services or indicate payment by check and the mailing address.
  • Please hold the days and times you indicated on your form.
  • All services requested will be provided on the same date.  While we do our best to arrange the services in the same window of time, this is not always possible.
  • All services require payment in full before a block of time for the services requested is arranged.
  • Confirmation of the day and block of time for the services requested will be sent the week of January 9th.

 
We understand that immediate notification of a date and window of time is desired.  Because of the high volume of requested changes when ring times are announced confirmation emails are only sent a few days before the health clinics.  Please be patient. 
 
The form is designed so that you can change your preferences as your schedule changes, learn of ring times, etc.

NORTHWEST CARDIOLOGY CONSULTANTS
Basic (Auscultation) = $53 per dog
Advanced (non-medical Echocardiogram) = $220 per dog
 
Eye Clinic
Dr. Paul Scherlie, DVM, DACVO
NWVS Ophthalmology
Genetic Eye Screen/CAER exam = $40 per dog
 
Please note: Both veterinarians are providing non-medical appointments for puppies and adult dogs for the Orthopedic Foundation for Animals (OFA) certification.  You can schedule a non-medical service with either or both vets.  Submitting the results to OFA is your choice and at your expense per OFA fees.
